Universal Recycling Request for Assistance: Commercial/Multi-family

The undersigned further understand(s) and acknowledge(s) that under Section 18-5-209 of the Colorado Revised Statutes, it is unlawful for a person to obtain Town assistance, including benefits under the Town’s Universal Recycling Financial Assistance Program, by means of a knowingly false statement or representation, and that violation of that law is punishable by fine and/or imprisonment.
In providing the above information, applicant(s) declares the same to be voluntarily furnished. Applicant(s) hereby grant the Town of Frisco the right to request verification thereof through persons and/or entities disclosed and/or hereinafter disclosed. Applicant(s) declares the above information is true and accurate, and understood by the applicant(s).
I authorize the Town of Frisco and other necessary agencies to share pertinent information in order to better coordinate services pertaining to waste and recycle curbside pickup.
I understand that I may be asked for an interview to explain and answer any questions that the Internal Review Committee may have.
I understand that if the information in this application and accompanying documentation is incomplete, inaccurate, false or found to be non-verifiable, my Universal Recycling Financial Assistance will be terminated immediately.
